The redesigned program—launched on April 3, 2017— replaces the three-year recognition cycle with an annual check-in and reporting process in order to sustain recognition status. This document contains reporting requirements for the annual check-in for practices with an annual reporting date from April 3, 2017 through September 30, 2018.
