This publication contains complete instructions for administering the Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey for PCMH (Patient-Centered Medical Home). It includes technical specifications, survey questionnaires, and standardized protocols required for data collection and accompanying correspondence.

Update: Because of the limited number of changes to the Specifications for the CAHPS PCMH Survey, NCQA will not issue a 2018 version of the specifications. Organizations should use the HEDIS 2017 Specifications for the CAHPS Survey for PCMH and the updates in the memo (enclosed with the epub) for HEDIS 2018 reporting.
